North West Disability Services is a registered provider of Support Coordination, if you need assistance to activate or implement your NDIS plan, or to locate mainstream or community supports, we can confidently guide you through this process. Our Support Coordination Team can help you by:

  • Providing guidance through the NDIS process.
  • Empower you to access and coordinate the supports in your plan.
  • Work together to identify goals and how to achieve them.
  • Select the best services and providers to meet your needs.
  • Help you participate more fully in the community.
  • Support you with your NDIS planning review meeting.
  • Offer support in points of crisis.
  • Provide carer and/or parent training.
